Complete the Wellness Assessment in Castlight
Complete the Wellness Assessment in Castlight between January 8, 2024 and December 20, 2024 and save $300 on 2025 medical contributions ($600 if your spouse/partner completes it, too). You’ll also earn 125 Castlight points!
HOW THE ASSESSMENT WORKS
- It will only take about 20 minutes to complete the assessment in Castlight.
- Select three health goals to personalize the assessment based on what you care about.
- Answer the questions tailored to your health goals.
YOUR HEALTH NUMBERS
Your personal health numbers include: blood glucose, blood pressure, triglycerides, HDL and LDL cholesterol, total cholesterol, weight and waist circumference. Since the Wellness Assessment asks for a few health numbers, it is encouraged to know your numbers. A Quest Health Screening is one way to know your numbers before completing the Wellness Assessment. If you have your health numbers from a different screening, like with your personal physician, you may use those to help you complete the Wellness Assessment.
WHO IS ELIGIBLE
All U.S. employees are eligible to create a Castlight account and earn rewards, regardless of enrollment in an Abbott medical plan. If your spouse/domestic partner is covered on an Abbott medical plan, they can create an account, too!
Newly hired U.S. employees and their spouses/domestic partners are eligible to complete the Wellness Assessment to earn medical plan contribution savings in 2025, not 2024. However, you will need to first complete your 2024 New Hire Enrollment within your first 31 days of your date of hire. Castlight will receive your eligibility approximately 7-10 business days after submitting your elections.
If you will be covering your spouse/domestic partner under the Abbott medical plan for the first time in 2025, they can create a Castlight account after 2025 Open Enrollment ends in the fall.
YOUR PRIVACY IS ALWAYS PROTECTED
All of the information in your Castlight account is completely confidential. Abbott will not have access to your personal health information. One of the reasons Abbott partners with Castlight, an independent company, is to help you feel confident personal results will not be reported to Abbott.
