Learn Your Numbers

Abbott offers various ways to get a free, confidential health screening. The screening is also available to your covered spouse/domestic partner and is designed to give you a picture of your health. Plus, it provides the information you need to complete the Wellness Assessment, which can lower your 2019 medical contributions by up to $600.

Get your health screening now until August 31, 2019, but consider doing so earlier so you can use your numbers to complete the Wellness Assessment by December 21, 2018.

Go to your Jiff account and find Health Screening in the Progress tab. In addition to gaining valuable information about your health, you’ll also earn 250 Jiff points that you can redeem for gift cards and more!

2 SCREENING OPTIONS

  • From a Quest Patient Service Center
    Free screenings for you and your covered spouse/domestic partner are available at a Quest Patient Service Center near you. Use your Jiff account to make a Quest screening appointment. In Jiff, select Progress > Health Screening.
  • From Your Physician
    Use your health numbers from your annual check-up with your personal physician. You can self-report your numbers in the Wellness Assessment. If you want the 250 points for getting your health numbers, you will need to have your physician submit the Physician Results Form to Quest.

    To download your personalized Physician Results Form, go to your Jiff account and select Progress > Health Screening. Follow the instructions to select a screening option and you’ll see the option for the Physician Results Form.

REPORT YOUR NUMBERS IN YOUR WELLNESS ASSESSMENT

You can now self-report the numbers from your health screening from Quest or your doctor. This means when you complete the Wellness Assessment, you’ll need your health numbers or can select “Not Sure” to proceed without them. Your numbers from Quest will update to your completed Wellness Assessment automatically. You can visit your Wellness Assessment later to review how your personal feedback changes based on your numbers.

The Wellness Assessment asks for health numbers like blood glucose, triglycerides, blood pressure, HDL, LDL, total cholesterol, weight and waist circumference.

HOW TO PREPARE FOR YOUR SCREENING WITH QUEST

Make sure you’re prepared for your health screening so you can get the best results – carefully read the requirements below:

  • You must fast 9-12 hours prior to the screening.
  • Take prescribed medications as you normally would.
  • Drink plenty of water before your appointment as this helps with your blood draw, but do not have any other beverages (like coffee, juice, or soda).
  • Your height, weight, waist measurement, blood pressure and a blood sample will be taken. The blood draw will be venipuncture.
  • Wear a loose-fitting shirt so that you can roll up the sleeve. This will make it easier for the technician to take your blood pressure and blood sample.
  • Allow 15-20 minutes for your appointment.

WHAT TO EXPECT AFTER YOUR SCREENING

  • ​You will see the results in your Jiff app 3-5 days after your screening. In Jiff, select Progress > Health Screening > View Results.
  • You will receive a personalized report from Quest by mail in 2-3 weeks to your home.
  • Your health numbers will not be pre-populated in your Wellness Assessment questionnaire, but they will connect to your completed Wellness Assessment.

IT’S FREE AND CONFIDENTIAL

There is no cost for you or your covered spouse/domestic partner to get the health screening. The health screening is completely confidential and managed by Abbott’s wellness partners. Abbott never sees individual results.

WHO IS ELIGIBLE

All employees and covered spouses/domestic partners who are enrolled for 2019 medical benefits are eligible to earn rewards by getting a free health screening.

NEWLY HIRED EMPLOYEES

Newly hired employees and their spouses/domestic partners are eligible to complete the health screening. However, you may need to wait to register for 3-4 weeks from your date of hire so Jiff can receive your eligibility. You have until August 31, 2019, to complete the screening. ​​

YOUR PRIVACY IS ALWAYS PROTECTED

All of the information in your Jiff account is completely confidential. Abbott will not have access to your personal health information. The program is administered by Jiff, an independent company, and personal results will not be reported to Abbott. ​
