Get a Free Flu Shot

Getting a flu shot is one of the best ways to protect yourself and your family from the flu. Protect yourself and your covered dependents with a flu shot administered by your physician or convenient care clinic. Abbott medical plan participants and their covered dependents can visit their in-network physician or an in-network convenient care clinic for their flu shot. If you go out-of-network, your flu shot is covered once you meet your deductible and pay coinsurance. If you’re in the HIP PPO or Traditional PPO plans, call the Abbott Care Coordinators at 888-614-1011 to learn more and access your free preventive benefits.

PROOF OF VACCINATION

If you participated in an Abbott onsite flu shot event in 2023 and need proof that you were vaccinated, you may request a flu vaccination receipt by sending an email to Affiliated Physicians at myresults@affiliatedphysicians.com. When you send an email, provide your first and last name, the location and date you got the flu shot, and your Abbott UPI. Please allow up to 5 days to be able to provide verification.
