HOW TO EARN A 2027 WELLNESS CREDIT
Complete a single qualifying preventive care visit between October 1, 2025 and September 30, 2026, to save $300 on 2027 medical plan contributions (or $600 if your covered spouse/partner completes a visit, too).

Why do I see December 31, 2026 listed as the deadline in Castlight?
Castlight shows the deadline for Castlight to receive verification the preventive care visit is completed. This verification process varies by medical plan – see details below.
NOTE: Employees and covered spouses/partners may still complete the Health Assessment or Quest Biometric Health Screening, but they are NOT activities to earn the wellness credit toward medical plan contributions.

Your Castlight account will automatically verify when the 2027 wellness credit is earned. Preventive care visit between October 1, 2025 and September 30, 2026 qualifies. - BEFORE SEPTEMBER 30, 2026
Schedule an eligible preventive care visit with a Provider. Contact your Abbott Care Coordinators for any questions about preventive care visits coverage and using in-network providers. You may also create an account with Teladoc Primary360 and book an Annual Checkup appointment.
Your Provider will submit the claim to your medical plan for payment, which can take up to 90 days. Please note: Given the time it may take for a claim to fully process, you must complete a visit by September 30, 2026, so it can be processed by December 31, 2026, to qualify for a 2027 wellness credit. Contact your Abbott Care Coordinators for any questions. - BY DECEMBER 31, 2026
After the claim is processed for payment, completion of the visit will be verified with Castlight on your behalf by December 31. You do not need to submit anything to Castlight.
DIDN’T USE YOUR ABBOTT MEDICAL PLAN AND/OR NEWLY ELIGIBLE in 2026
If you completed a qualifying preventive care visit, even under a different medical plan, between October 1, 2025 and September 30, 2026, contact your Abbott Care Coordinators for assistance and details on the documentation needed. Once the documentation is provided to your Abbott Care Coordinator, it will take 7-10 business days for evaluation. If a qualifiable preventive care visit was completed, completion of the visit will be verified by the Abbott Care Coordinator with Castlight on your behalf.
Surest
- Schedule an eligible preventive care visit with a provider. Contact Surest Member Services for any questions about preventive care visits coverage and using in-network providers.
- Complete an eligible preventive care visit before September 30, 2026. Provider will submit claim to your medical plan for payment, which can take up to 90 days. Contact Surest Member Services for any questions.
- After the claim is processed for payment, completion of the visit will be verified with Castlight on your behalf by December 31. You do not need to submit anything to Castlight.
- Your Castlight account will verify when the 2027 wellness credit is earned.

Is the credit showing in Castlight for 2026 or 2027? How do I know if I received the 2026 credit?
The wellness credit you see in Castlight now is for your 2027 credit. If you earned a 2026 wellness credit, it is listed on your paycheck.
How long does it take for my 2027 wellness credit to show up?

Can my spouse/domestic partner earn a 2027 wellness credit?
Yes! If your spouse/domestic partner is covered on an Abbott medical plan in 2026, they are eligible to earn a $300 medical plan contribution in 2027. That means, if both you and your covered spouse/partner completes a visit, you can earn $600 on 2027 medical plan contributions.
Note: Castlight will only display an individual’s wellness credit total.
Why do I see December 31 listed as the deadline in Castlight?

If I am not enrolled in an Abbott medical plan now, but I elect coverage for 2027, can I still earn the 2027 wellness credit?
No. Employees and their spouse/domestic partners enrolled in a 2027 Abbott medical plan are eligible to earn the 2028 wellness credit toward their 2028 medical plan contributions.

2026 WELLNESS CREDIT
The timeframe to earn your 2026 wellness credit has passed. If you took action to complete an eligible preventive care visit between October 1, 2024 and September 30, 2025, and do not see your wellness credit in your paycheck or if you have additional questions, contact Abbott Health Benefits at 844-306-9222.

All of the information in your Castlight account is completely confidential. Abbott will not have access to your personal health information. The only information that will be shared with Castlight is that a preventive care visit was completed. No details on the type of screening or visit are ever shared with Castlight or Abbott.
