HOW TO EARN A WELLNESS CREDIT

There’s a new way to earn a wellness credit. Complete a single qualifying preventive care visit between October 1, 2024 and September 30, 2025, to save $300 on 2026 medical plan contributions (or $600 if your covered spouse/partner completes a visit, too).

QUALIFYING PREVENTIVE CARE VISITS:

• Well Visit with a Primary Care Provider (PCP)
• Breast cancer screening
• Cervical cancer screening
• Colorectal cancer screening
• Osteoporosis screening
• Prostate cancer screening

NOTE: Employees may still complete the Health Assessment or Quest Biometric Health Screening, but they are NOT activities to earn the wellness credit toward 2026 medical plan contributions.

ACT NOW TO SAVE IN 2026

To complete a preventive care visit by September 30, 2025, you’ll want to take action now. Schedule an eligible preventive care visit with a provider. Partner with your medical plan to identify in-network providers.

Blue Cross Blue Shield and UnitedHealthcare

• FROM JAN. 1, 2025 – DEC. 31, 2025
  Depending on when your claim is processed, your Castlight account will verify when the 2026 wellness credit is earned.
• NOW
  Schedule an eligible preventive care visit with a provider. Contact your Abbott Care Coordinators for any questions about preventive care visits coverage and using in-network providers.
  • If you completed an eligible preventive care visit from October 1, 2024 – December 31, 2024, an Abbott Care Coordinator can confirm if your visit qualifies and ensure the claim was submitted and processed correctly; note it can take up to 90 days for your claim to be processed.
• BEFORE SEPTEMBER 30, 2025
  Complete an eligible preventive care visit. Your Provider will submit the claim to your medical plan for payment, which can take up to 90 days. Please note: Given the time it may take for a claim to fully process, you must complete a visit by September 30, 2025, so it can be processed by December 31, 2025, to qualify for a 2026 wellness credit.
• BY DECEMBER 31, 2025
  After the claim is processed for payment, completion of the visit will be verified with Castlight on your behalf by December 31. You do not need to submit anything to Castlight.

New Hires in 2025
As a new Abbott benefits participant, you and your family have access to a wide range of benefits that support your health and well-being, including free preventive care as part of your medical plan! If you had previously completed a preventive care visit with a different medical plan, see details below to see if it qualifies to earn the 2026 wellness credit:
• Contact your Abbott Care Coordinators for assistance and details on the documentation needed. Once the documentation is provided to your Abbott Care Coordinator, it will take 7-10 business days for evaluation.
• If a qualifiable preventive care visit was completed, completion of the visit will be verified with Castlight on your behalf.

Surest

• Schedule an eligible preventive care visit with a provider. Contact Surest Member Services for any questions about preventive care visits coverage and using in-network providers.
• Complete the visit before September 30, 2025. Provider will submit claim to your medical plan for payment, which can take up to 90 days.
• After the claim is processed for payment, completion of the visit will be verified with Castlight on your behalf by December 31. You do not need to submit anything to Castlight.
• Your Castlight account will verify when the 2026 wellness credit is earned.

Kaiser

• Schedule an eligible preventive care visit with a provider. Contact Member Services for any questions about making an appointment.
• Complete the visit before September 30, 2025.
• Log into your Castlight account to attest you completed a qualifying preventive care visit between October 1, 2024 and September 30, 2025. Go to Castlight > Benefits > Kaiser to complete the attestation steps by December 31, 2025.
• Your Castlight account will update and verify the 2026 wellness credit is earned.
NEW HIRES & NEWLY ELIGIBLE IN 2025
• Already completed a preventive care visit?
  If you completed a qualifying preventive care visit, even under a different medical plan, between October 1, 2024 and September 30, 2025, log into your Castlight account to attest you completed the visit. Go to Castlight > Benefits > Kaiser to complete the attestation steps by December 31, 2025.
• Need to complete a preventive care visit?
  Please see the information above to schedule and complete a visit using your Kaiser benefits by September 30, 2025. Then, follow the steps to complete attestation by December 31, 2025.

Cigna Global Health

• Log into your Castlight account to attest you completed a qualifying preventive care visit between October 1, 2024 and September 30, 2025. Go to Castlight > Benefits > Hawaii HMSA to complete the attestation steps by December 31, 2025.

WHO IS ELIGIBLE

All U.S. employees are eligible to create a Castlight account and earn rewards, regardless of enrollment in an Abbott medical plan. If your spouse/domestic partner is covered on an Abbott medical plan, they can create an account, too.

Newly hired U.S. employees and their spouses/domestic partners are eligible to complete a qualifying preventive care visit to earn medical plan contribution savings in 2026, not 2025. However, you will need to first complete your 2025 New Hire Enrollment within your first 31 days of your date of hire. Castlight will receive your eligibility after submitting your elections. You will be able to create an account approximately 7 business days after that date. If you do not receive the “Your Account is Ready” email from Castlight after 31 days from your date of hire or the date of being added to Abbott Medical Coverage, please contact Castlight Support at 844-711-7113.

2025 WELLNESS CREDIT

The timeframe to earn your 2025 wellness credit has passed. If you took action to complete a Wellness Assessment by December 20, 2024 and do not see your wellness credit in your paycheck or if you have additional questions, contact the Abbott Benefits Center at 844-306-9222.

YOUR PRIVACY IS ALWAYS PROTECTED

All of the information in your Castlight account is completely confidential. Abbott will not have access to your personal health information. The only information that will be shared with Castlight is that a preventive care visit was completed. No details on the type of screening or visit are ever shared with Castlight or Abbott. One of the reasons Abbott partners with Castlight, an independent company, is to help you feel confident that personal results will not be reported to Abbott.
